You or anyone on your behalf, including family, hospital, family doctor, friend, neighbour, community group or government agency, can make a referral to Home Health by following the three steps below. Step 1 - Contact us Contact home health services in your area. See the list of Home Health services at the bottom of this page. Step 2 - Get assessed by a case manager If you feel you need services such as home support or access to an adult day program, or you wish to be considered for assisted living or residential care, a Home Health Case Manager will assess your situation. The case manager will review your needs, eligibility for services and costs. The assessment will take place in your home, in the hospital or in another care setting. If you require nursing or physical and occupational therapy services, a nurse or therapist may visit you in your home to assess your specific additional needs. How long do I have to wait before I will see a case manager? People who are eligible for Home Health services may be seen for an assessment within a day or two, or in several days, depending on the urgency of need. While every attempt is made to accommodate exceptional or urgent circumstances, it is important to remember Home Health services are not emergency services. Step 3 - Develop a care plan Following the assessment, an individualized care plan is developed and appropriate Home Health services are arranged as required. The goal is to help people with multiple needs live at home safely and to avoid unnecessary or premature admission to a hospital or care facility. What if Home Health services don’t meet my needs? If it is determined that Home Health services are not required, you may be directed to other community resources to meet your needs. Am I eligible?
