
Indigenous health liaisons are a key part of care teams, supporting health and wellness journeys for Indigenous clients and their families in hospital and community settings.
Indigenous health liaisons can:
- Help you navigate the health care system
- Support your transition from hospital to home
- Work alongside health care providers to support your decisions in care planning
- Connect you with an Elder or cultural advisor to support cultural practices
- Connect you with Indigenous community resources
All clients who self-identify as Indigenous or Aboriginal can receive support from an Indigenous health liaison.
