Home and Community Care offers a range of health care and support services in your home and community. Our goal is to support your independence, health and quality of life.
Hours of operation
Monday - Friday, 8:30 a.m. to 9:00 p.m.
Referral
Current clients should reach out to their local Home Health office listed below. Your health care provider may also recommend our services. For information or referrals, contact 1-855-412-2121 (toll-free, 7 days/week, 8:30 a.m. - 9:00 p.m.; statutory holidays, 8:30 a.m. - 4:30 p.m.).
Fees
Some services will have no cost to you, while others will require you to provide your tax information to access services.
Language (s) Offered
Translation services available upon request.
