Referral
Individuals are referred to the clinic by a neurologist from the Jim Pattison Outpatient Care and Surgery Centre Stroke Clinic. Patient will be contacted by clinic following neurologist referral.
What to bring
Please bring all your medications to the appointment. If you have a blood pressure machine at home, please monitor your blood pressure for 7-10 days prior to the appointment and bring down written readings to the appointment with you. If you have diabetes, and you are measuring your glucose readings, please write down 7-10 days of glucose readings prior to the appointment. Vary the time of day you measure, sometimes before a meal, and sometimes 2 hours after a meal.

What is a pharmacist led stroke prevention clinic?
You are being referred here either because the cause of your stroke has not been clearly determined and we are awaiting tests to review or you are being referred to help reduce your risk of more strokes by addressing any of your underlying risk factors. Common risk factors for stroke are high blood pressure, diabetes, high cholesterol, diet ,and lack of physical activity and smoking. Patients are discharged back to their family doctor’s care when we get you to the guideline recommended goals to prevent another stroke.

I have a general practitioner (GP), why do I need it?
We do not replace your family doctor. We notify your family doctor of all changes made to your medications via written documentation. Optimizing your vascular risk factors will help reduce your chance of having another stroke.

How long will it take?
The first appointment is one hour. Please ensure you are on time to allow us sufficient time to provide best patient care.

How do I measure my blood pressure (BP)?
Did you know that only 10% of blood pressure machines sold are validated machines?
