Eligibility
- Children and teens six to 17 years
- Body mass index (BMI) for their age is greater than the 97th percentile
- BMI for their age is between the 85th and 97th percentiles and they have specific health concerns
- The parents/caregivers and child/teen must be interested in making healthy living changes, and able to attend appointments and group sessions
Referral
Only available by referral from a physician or nurse practitioner.
