Physician referral and requisition forms are required. Once a requisition is received, we will contact the patient with an appointment date and time. Please confirm the appointment as soon as possible. No drop-ins.
Confirming your appointment
If your appointment has recently been scheduled, you must call 604-523-8886 to confirm your appointment. When you call to confirm your appointment, please leave your name, personal health number, phone number and date of MRI.
Please note: If you do not confirm your appointment, your appointment may be cancelled and given to another patient.
Changes or cancellations to your appointment
You may email RCHMRI@fraserhealth.ca for appointment changes or cancellations. We will send you an email communication to confirm your consent to which you must reply “Yes, I consent” before we can send you a response.
