The White Rock South Surrey (WRSS) – Primary Care Network (PCN) collaborates directly with the patient's most responsible provider (MRP) and offers support for medication management, resource navigation, and connections to community services for pain management.
All chronic pain services are available via referral from a physician/nurse practitioner attached to the WRSS PCN.
Chronic pain group sessions are offered in-person and online:
- Three sessions, 90 minutes each:
- Introduction to chronic, non-cancer pain.
- Non-opioid medications.
- Opioid medications.
- These sessions include education on self-management skills and strategies, as well as a nurse and pharmacist review of medication options.
Additional chronic pain services include:
- Nurse assessment on initial contact with the program, with the option of a home visit.
- Short-term nursing support on the completion of the chronic pain education series to aid in goal-setting, development of a pain flare-up plan and connection to other resources.
- Pharmacist assessment for an individualized medication review.
- Referrals to further pain education within Fraser Health, or self-paced virtual and in-person resources from community partners.
Access
Eligibility
- Most responsible physician (MRP) must be a WRSS PCN provider.
- Participants must be adults, age 18 and older, living with chronic, non-cancer pain.
- Pain must persist for greater than three months that may arise from long-term medical conditions not expected to improve (e.g., osteoarthritis, scoliosis, multiple sclerosis, fibromyalgia, lower back pain) or pain that persists beyond the expected time of healing.
Referral
- Individuals must be referred by their WRSS PCN-attached provider.
- Referrals should be sent through Pathways BC.
