The Community Pain Self-Management Program is a comprehensive, eight-week virtual pain program.

What kind of services are available?

Eight-week pain self-management program provides:

  • Group sessions on pain management to learn and build self-management skills (i.e. breathing techniques and mindfulness) to decrease pain and improve quality of life. Topics include:
    • Pain science, pacing, managing thoughts and emotions, stress management, sleep wellness, nutrition and pain, safety with movement, self-compassion, and more.
  • Group gentle movement activities.
  • Individual virtual appointments as needed (with a nurse, physiotherapist or occupational therapist).
  • Assisted referrals to more pain, mental health and substance use services where applicable.
  • Awareness and assistance connecting to community resources to support ongoing pain care.
  • An individualized discharge plan created with the client. This will be shared with the primary care provider for ongoing support and care.

The commitment to complete this program is 2.5 hours per week for eight weeks.

Access

Eligibility

People ages 17 years and older with persistent pain, who are interested in self-management skill building and able to participate in group learning. Clients will also need access to the internet and have either a mobile device (phone or tablet) or computer to use.

Please note: The eight-week program does not offer individual pain specialist consults or interventional management. Please see Pathways BC to search for a variety of pain services located near you.

Referral

Adults living in the Fraser Health region can be referred to the program by their primary care provider or the Fraser Health Mental Health and Substance Use Services. Clients must have a Primary Care Provider for referrals to be processed.
