Supportive service in a safe, home-like integrated long-term care settings for people ages 19 years and older who require longer term specialized mental health support and rehabilitation.

The team provides ongoing mental health treatment and rehabilitative services to promote the individual’s skills and ability to manage their lives as independently as possible. The team works closely with the individual by encouraging and supporting individual’s goals for recovery and moving to a more independent-living environment. 

The average length of stay is 24 to 36 months.

Referral

Access to these services are generally through a hospital setting or a health care professional.

In some cases a family doctor may be able to refer an individual to some tertiary programs.
