Integrated health support teams in Fraser Health are part of the Ministry of Health Integrated Support Framework initiative to support people at risk of or experiencing homelessness.
Health care, social services and non-profit housing providers deliver wrap-around care for adults 19 and older living in market housing who have mild to moderate health and social needs.
Supports include a rental subsidy administered through BC Housing and clinical supports and health system navigation provided by Fraser Health.
Registered Nurses and Registered Social Workers provide client-centred, trauma informed health supports and provide health system navigation to other services such as Indigenous Cultural support teams and food security programs. Housing and health care plans are done collaboratively with the client and service provider.
Services are delivered five days a week. The programming is culturally responsive, utilizes a harm reduction approach and is inclusive of personal choices, natural supports and diverse cultures and lifestyles.
