Please return all your open and unopened doses of publicly funded influenza vaccine received from Public Health back to your local public health unit by June 17th, 2019.

This is vaccine you would have received from your local public health unit or, for acute sites, from BCCDC directly.

Complete this form and include this with your package of returned influenza vaccine.

If you do not have any doses to return for a specific vaccine, please indicate “0” in the “Total Doses Returned” column for that product and return this form to your local public health unit.
