Vaccine harvest letter to immunizing physicians, pharmacists, residential care and acute

To immunizing primary care providers,

Please return all your open and unopened doses of publicly funded influenza vaccine received from Public Health back to your local public health unit by May 10, 2023. Vaccine does not need to be returned under cold chain.

Complete the table in the documents attached below and include this with your package of returned influenza vaccine.

If you do not have any doses to return for a specific vaccine, please indicate “0” in the “Total doses returned” column for that product and and return this form to your local public health unit. Partial vials should be recorded as 5 doses.
