Primary Care Networks (PCNs) are clinical networks of primary care service providers located in a specific geographical area. These networks support patients through a team of health care professionals, including family doctors, nurse practitioners, registered nurses, pharmacists, social workers, midwives, mental health specialists, Indigenous providers and community health workers.

In the Fraser Health region, some Primary Care Networks offer pain-specific support programs tailored for those living in their communities.
