We provide a range of integrated home and community-based professional and caregiver services to help you remain independent at home for as long as possible. No matter where you find yourself on your care journey, we will help connect you to the right services.
All community-based professional services will help coordinate and connect you to programs and services, as needed, including other health services and community resources.

New clients
Make a telephone appointment with our Fraser Health Access Line: Home and Community Care (previously called the Home Health Service Line). There are 2 ways to do this:

We are open 7 days a week from 7:00 a.m. to 9:00 p.m., 365 days a year.

Fees
Some services are free, while others include a cost based on your after-tax income.
