The medication management program consists of home visits by pharmacists.
Our medication management program offers personalized home visits conducted by experienced pharmacists. During these visits, your current medications will be reviewed, unnecessary medications will be discontinued, and tailored aids such as blister packs will be introduced as needed. Moreover, our team will provide valuable recommendations to you and your family physician.
This service is specifically designed for older adults living with complex medication regimens, typically involving six or more medications and those at risk of drug interactions. Individuals with conditions such as kidney or liver failure, dementia, confusion, or recent hospital discharge are also prime candidates for our program.
Additionally, there is a medications return program to ensure the safe disposal of expired or unused medications. Proper disposal is crucial in preventing accidental consumption by children and misuse by others. It also minimizes environmental impact by keeping medicine out of landfills and water supplies. Unused medications can be returned to any pharmacy for safe disposal.

Eligibility
Our program is available for individuals aged 65 and older who are at risk of medication misadventures, including those recently discharged from the hospital or managing complex medication regimens. Long-term care clients are excluded from this service.
