Connecting older adults to innovative services in their community to support health and social well-being.
A local seniors' community connector will meet with you to help develop a personalized wellness plan to support your healthy aging journey. They will connect you with available community resources and liaise with your family practitioner for additional support.
Seniors' community connectors help connect you to services including, but not limited to:
- Social activities and programs
- Physical activity and exercise programs
- Meal delivery and nutrition programs
- Family and caregiver supports
Seniors' community connectors also help participants access:
- Transportation support
- Language or translation support
- Emotional support
- Culturally appropriate programming

Speak to your health care provider or call 2-1-1 about how you can be referred to this service.

An example of social prescribing in action:
1. An older adult learned from their doctor that they have early dementia.
2. They told their primary care provider they want to remain independent and living at home for as long as possible. They’re worried about depending on others for transportation when they have to give up their drivers’ license.
3. Their primary care provider told them about ‘social prescribing’ and referred them to a seniors’ community connector for help and support with their non-medical needs – like physical and social activities, grocery shopping and transportation.
4. The seniors’ community connector partnered with them to develop a wellness plan focused on their wellbeing.
5. This plan included applying for HandyDART transportation, setting up meal delivery and light housekeeping services and enrolling in a program to stay physically and socially active in their community. They were also connected with an Alzheimer’s Society support group and a grief counsellor.
