Most people already receive services in their home or community from Fraser Health before requiring assisted living or long term care. If you are receiving service and have a community case manager, please speak with them about your further care needs.
You may also contact the Home Health Service Line at 1-855-412-2121 (open 7 days a week, 8:30 a.m. - 9:00 p.m.) to request an assessment. A community case manager will come to your home and complete an assessment to determine care needs.
